Someone could do a game from scratch and have the buttons be the d pad and maybe the home button, etc, and have you strum on the touch screen?

BTW, that one is quite mild form of WSOD. I have one similar piece myself and found that it can be almost perfectly readable, you just need to avoid some colors (like pure black, blue, ...). Looks...

No. Any software you install cannot cause WSOD (at least directly)
Yes. WSOD can happen when device is powered on or rebooted. For flashing you sadly need to do this. You are completely safe...

The WSoD has to do with an power problem on the LCD chip, although I'm not sure what exactly causes this failure. Some people have got it by plugging in the AC adapter, then immediately trying to...
