can you make a video please ,how to use it ?

Playing videos? Sorry but this is just a widget to control other players, it's not a player itself.

What happens when you launch it from command line?

Is the desktop file on your device? (/usr/share/applications/hildon-home/packedmediawidget.desktop)

Do you have any other qt homescreen widget installed & working?

I'm sorry, but right now the icons are embedded in the application.
