Photos keep duplicating
 

Hi all,
This started last week. Whenever I take photos from my camera, and check in Photos there multiple instances of the said photo.
If I delete all but one all photos get deleted with message "Image no longer available"
If I delete only one photo more are spammed.

And another problem I noticed is that one of my folders containing songs called Music doesn't get indexed in Media player. I tried many thinks like forced to start indexing again reboot etc.

The last thing I installed was SpeedPatch from Karam.
I unistalled it but to no avail.
Please help.
