it certainly does, I´ve just replaced (renamed) 5 icons in nano, saved it with CTRL + O and change is instantly visible without any need for reboot or restart the homescreen...
just open the folder on the homescreen (if you have the app in a folder) and it switches the icon itself...

edit: avoid CTRL + O for saving changed icon, press CTRL + X + Y instead...the latter always works
