qgvdial seems to have stopped working for me just around today - keeps complaining that I have an incorrect username/password, though I'm sure they're correct. I have two-factor authentication turned on, and it does get to the stage where it asks me for the security token - but even after typing this in correctly, it complains about the invalid username/password.
