Does dmesg show some useful output at that point, which might give a hint as to why it's dying?
Alternatively, you might want to look and see what /etc/init.d/bluez-utils actually starts, and try starting them from the command line (and see if they can be made to remain attached, rather than daemonising, so you can see their output).
