You need to manually tweak the config to make the $5.99 plan work, as none of the presets is right for it.

Either use the T-zones setting, and fix the proxy (it comes with a leading zero, which breaks some stupid bug), or (better, but may not stay around) use the T-mobile setting, and change the APN to epc.t-mobile.com.
