Given that a reflash fixed it, and it was brand new, it seems that the original flash image on the device must have been somehow corrupted. Based on Qole's description of his friend's experience, it sounds like the flashing process isn't 100% error-free (although presumably it's really close to 100% error-free or we'd hear a lot more about such problems).
