I'd say drop the older version, then send the new package into some sort of administrative approval situation where an admin will be required to examine the update and confer with the maintainer before the package can be promoted (as opposed to resetting the votes entirely).

Can we detect that an uploaded package is an update to a package in the queue and require the uploaded to enter a paragraph or so explaining the update?
