1. application rebate form (filled out)
2. invoice receipt copy
3. upc code with ime on the box ( can it be a copy or do we have to cut it out of the box?) what about if returns or something..

Cutting it off the box is going to make returns tricky. I guess you need to decide if its worth the US$50 to you
