Had excactly the same issue as #1.
Then after i charged it fully with wall charger, i connected it to my computer with usb and charged it for another half hour. And now it charges fully everytime with wall charger after that (1222mAh and it reports my battery is max at 1250mAh)

It has something to do with the pc usb cable charging at a lower rate so the battery is more acceptable to filling battery up more.
