As everyone before me has pointed out, you need to have a SIP or skype account with a usable outgoing number.
I have two SIP accounts (gizmo5 and sipgate) and skype with a valid number.
Callback to these accounts works from three applications: "TheOneRing", "Dial Central" and "qgvdial", callout works in qgvdial.

Call out works for any voip (skype/sip) in which your callerID is set correctly and matches the information in your Google voice phone settings. Gizmo callouts have _never_ worked for me - I am guessing thats because the callerID of the gizmo account makes Google voice think that it isn't an acceptable number (or something).

Also: From random reports on the Intarwebs, I heard that Pidgin on Linux can receive incoming Google Voice calls on gtalk accounts. I have not checked this out, but if it is true, then I wonder why it wouldn't work even with Pidgin on Maemo and also the telepathy handlers in Linux and Maemo.
