It happens to me occasionally and is solved by removing the headphones and restarting media player.

You're right! WTH? That's not really a fix. Well it might be under OSS, but this is silly. Is there a permanent fix?

I just happened to crank up the N900 today while having a headphone directly plugged in, and noticed that the channels were reversed. Tried a track in QSpot, same. Via Bluetooth - fine. So I can only blame the jack.
