Never, and I have extras-testing and extras-devel activated.
The only annoyance I've had was when upgrading to PR 1.1 I had to uninstall quake and move the apt-cache from rootfs

Never since Dec 5th, but there's no option for that

(Plenty of extras-devel apps, too. Did nearly brick it once, but that was my fault.)

As it stands. this poll is extremely misleading. It does everything it can to dissuade you from voting 0.

I flashed twice, once purely for experience and once because it seemed like a good idea. None because I had to.
