Same issue here, the alarm app crashed a few days ago, probably around the same time as on sygys device. I just took notice of the camera issue yesterday. It crashed immediately after having started it. Also the flashlight status bar app indicated that another application is using the camera and it can't turn on the LED light. The FCamera displayed the infamous gray screen.
I tried reinstalling omap3camd as suggested above. That fixed it somewhat. The camera app was starting again. But the image had a green tint and the auto-focus did not work anymore. Things are different in the FCamera app, everything worked there. My best guess is that some configuration files got jumbled up.
So finally I created a backup, re-flashed the PR1.3 image and restored the backup. Everything is fine again. Either way, I still wonder what has caused the issue in the first place.

any ideas to this issue? I have tried everything listed in this forum and can't find what could be wrong.

I am suspecting it is some compatibility issue with some other app.

Here is easy way to fix this.. You do not need to remove power-kernel!
Make sure extra-devel is on.
xterminal:
Code:

Root
apt-get update
...wait..
apt-get remove fcamera
.....wait..
reboot
When it run agen.
Xterminal:
Code:
root
apt-get install fcam-drivers
...Wait..
apt-get install fcamera
....Wait...
reboot
hope this will help.

Oops. I had the camera not working after updating the power kernel and tried this fix. I think I bricked my phone instead.

Looks like I'm gonna try reflashing the kernel.

One note, when I did apt-get remove fcamera, apt told me that it was removing several kernel-related packages. I was on auto-pilot, and should have just said "no."

Update: flashed just the kernel and we're up and running again.

In my case (PR1.3, 2.6.28.10power46) I just did:

sudo gainroot
apt-get update
apt-get install fcam-drivers

and I am shooting again! :-)
