For me, I followed this guide and reinstalled the kernel. The camera worked for a day and then the Operation Failed message came back the next day. what I did was I uninstalled BlessN900, restated, and it has been a few days with the camera working. Let me know if this works for anybody else.

You have to navigate to the directory where you saved the fcam drivers before you can dpkg them.

some reasons i figured out why i have an "Operation Failed" camera.

-outdated fcam drivers
-unstable kernel power (i believe before maemo v28 and v43-45)
-with stable kernel power, but very low voltages settings
-corrupted/read-only microSD card. (if you are using 32GB microSD, it has a 1 out of 2 chance that it will work, coz i have 3 and only one od them works, and class 4).

just sharing

Ope X terminal and write:
sudo gainroot (I'm not sure about root access)
apt-get update
apt-get upgrade

If you are with titan 45 you will get somehow automaticly new fcam drivers and It will solve the Operation failed
Big thx. Problem solved after reboot!

Best solution so far. I uninstalled BlessN900, rebooted and that was it: camera is working again.

Thanks to Jakiman for this help.

I downloaded the updated fcam drivers from here (put them in MyDocs)
http://n900.tannerlab.com/kernel/fca....7-1_armel.deb

Then installed them with xterm:

cd /home/user/Mydocs
dpkg -i fcam-drivers_1.0.7-1_armel.deb

Camera working again.
Thanks a lot, this fixed my Camera Problems after installing the power kernel 46 on PR1.3
