I came upon this form at onsale.com. It looks like someone scanned it. Has anyone tried to use it and did it work? It's for N series devices for
US residents.

[edit] I just bought an N800 from Amazon. I'll submit this form to Nokia and see what happens...

On the bottom of the form it says mail-in forms that are mechanically reproduced will not be accepted. Does anyone have a place to get originals of these?
