I see missed calls from "Voicemail" when I have a voicemail waiting for me. It seems to only happen when I have missed the call with my phone on, though. In other words, when I don't answer an incoming call and it rolls to voicemail and the person leaves a message, I see their missed call as well as one from voicemail.
Also, as per your screenshot - I have a yellow icon next to voicemail too. But, as soon as I dial in to listen to my voicemail, that yellow icon disappears. I guess this is how the N900 voicemail indicator is implemented.
I'm on T-Mobile, and I've noticed this since before PR1.1.

I get missed call notifications, but the number that claims to have called me is from 'my voicemail' number. No call actually occurred.
Only a single and correct number is assigned as my voicemail and also no incoming call actually happened as verified by my at&t account activity. This happened last week and again today.. strange
I filed a bug for this

Somehow the N900 thinks my voicemail is calling me (but did not)..
