I have been thinking about my problem with BT audio. My BT headset was broken already so my only possibility to test is my car's BT.
I came about a question:
Is there anywhere a file which tells audio which volume to use when the device is changed (no headset, cabled headset, BT headset, BT access profile...)

@JohnHughes: Can you test new version? http://atrey.karlin.mff.cuni.cz/~pali/pa/new/
First two calls rang my ringtone.
Rebooted.
Got the default nokia tone.
Called again - no ring.
called again - my ringtone
called again - my ringtone
called again - no ring
called again - no ring.
Conclusion - the cssu3 version is buggy
Reverted to cssu1 version
1st two calls worked
rebooted
1st call worked
2nd call worked
3rd call worked
4th call worked
conclusion - the cssu1 version is ok.
